In accordance with the State of Florida Department of Health Public Health Advisory, individuals presenting for their first dose of the COVID-19 vaccine must be able to demonstrate Florida residency. Please click here to find out how to prove Florida residency.

1. Verify you are a current patient and on the State priority list and a Florida Resident
2. Complete the Covid-19 Vaccine Pre-Registration Form and wait for our office to contact you
3. Once our office has contacted you and you are scheduled, complete the Covid-19 Vaccine Consent Form prior to your appointment

Yes, our physicians strongly urge most patients to receive the Covid-19 vaccine. Tell your vaccination provider about all your medical conditions, including if you have any allergies, have a fever, have a bleeding disorder or are on a blood thinner, are immunocompromised or are on a medicine that affects your immune system, are pregnant or plan to become pregnant, are breastfeeding, or have received another COVID-19 vaccine.
Yes, the CDC recommends that you should be offered the vaccine even after infection. However, you should wait until the illness has resolved and you have met the criteria to discontinue isolation.
